Our herbal consultations require an in-depth intake form to be filled out and returned at least 24 hours before your scheduled consultation in order to be thoroughly assessed. The intake form is available for download using the link below, please fill out every section honestly and completely. You may either email or mail the form back upon completion. If you are unable to download the form, please mention so in the inquiry and an email will be sent to you containing the intake form from diamondrayhealingarts@gmail.com. Please be sure to add this address to ensure delivery success. At your consultation, we will discuss the assessment and co-create a custom blend specific to your body type, energetics (according to Ayurvedic and TCM systems), preferred form (spagyric, elixir, tincture, alcohol-free, dry herbs, etc), and daily habits.

We kindly ask for payment either before or at time of intake form submission, a lot of time is spent analyzing the intake form and generating various remedies to provide you with a smooth and efficient consultation.
